Dear Parents,
Elementary Schools
Students nine years of age and younger who have already received a first dose of the H1N1 vaccination will require a second dose. Please attend the clinic scheduled for your child's school of attendance if they are in need of a second dose (see schedule below).
Parents MUST be present when the vaccination is given. The vaccination is free.

The health department scheduled the community clinics as listed below.
H1N1 vaccinations will not be given at your childs school during the day.
- Parents MUST be present when the vaccination is given.
- Parents MUST complete a new vaccine consent form at the clinic site to update health history information, and list completed vaccinations received since October.
The vaccination is free.
